[Korus-aq] DC-8 Medical Clearance and Flight Participation Forms for KORUS-AQ

Dear KORUS-AQ Participant,

If you wish to be eligible to fly on the DC-8 when seats are available, please download and complete the necessary Flight Medical Clearance and Flight Participation forms attached to this email. If you know of others (e.g., leaders in your organization, members of the press, etc.) who would like to be considered, please forward this email to them.

Flight Medical Clearance forms: can be sent directly to Chief Flight Surgeon, Dr. Gregg Bendrick (Gregg.A.Bendrick at nasa.gov<mailto:Gregg.A.Bendrick at nasa.gov>) as instructed on the form.

Flight Participation forms: you only need to enter your name and complete Section 1. When providing your Organizational Contact, do not provide the name of a family member; it should be someone from your organization. Send the form to Jhony Zavaleta (Jhony.R.Zavaleta at nasa.gov<mailto:Jhony.R.Zavaleta at nasa.gov>) with a copy to Jim Crawford (James.H.Crawford at nasa.gov<mailto:James.H.Crawford at nasa.gov>). Include a short note about why you wish to be considered for a seat on the DC-8.
