[All] CAC card form

All,
I’m filling out the CAC card request form and checked the instructions (http://www.dmdc.osd.mil/smartcard/docs/1172-2_Instructions.pdf).  There are a couple weird things so it is worth reading this.  They do dates 2017Dec06 rather than 20171206 and they want the 9 digit zip code.

I guessed at a few things like….
30/31 I did not fill out since she put info in 36.
33/34 I used 2018Feb01 to 2021Jan30
37 Sounds like the signature is supposed to be notarized.  I asked Liz to check this before Dan sends the info on to her.
